Summary

This study tests a recovery navigation program for people aged 65+ who have survived a serious illness requiring intensive care. It provides personalized information and support to help patients and their doctors coordinate care after leaving the hospital. The goal is to improve recovery, reduce new disabilities, and prevent unnecessary return trips to the hospital.
