Summary
This study is testing a community-based program to help older African American adults in Houston better control their high blood pressure. It will compare two types of support: one led by pharmacists and another that adds help from community health workers. The goal is to see if this combined, personalized support can help people lower their blood pressure and stick to their medications over two years.
