Summary
This study looks at how older adults recover from an acute illness, comparing standard hospital care with a Hospital at Home program. It also tests whether a home-based exercise and thinking activity program after discharge helps improve physical and mental function. About 200 older adults from Navarre will take part, and researchers will measure their activity levels, physical abilities, and quality of life.
