Summary
This study focuses on making it easier for parents of children with multiple chronic conditions or technology dependence to care for them after a hospital stay. Researchers will check how well parents understand and follow discharge instructions, and track any health problems like readmissions within 30 days. About 282 parents and children will take part, with the goal of reducing errors and improving outcomes.
