Summary
This study tests if better communication between a hospital pharmacist and a patient's local pharmacy can reduce medication errors when patients go home. It focuses on adults taking seven or more medications who are being discharged from the hospital. Researchers will compare the usual process to a new one where the hospital pharmacist directly shares information with the local pharmacy to help them prepare prescriptions safely.
