Summary

This study looks at whether a hospital pharmacist sharing information with a patient's regular community pharmacist can reduce drug-related problems after discharge. It involves 150 adults who take 7 or more medicines and are leaving an internal medicine ward. The hospital pharmacist works with the community pharmacy, not directly with the patient, to make discharge prescriptions clearer and safer.
