Summary

This study looks at two things: first, how hospital care compares to home-based hospital care for older adults with an acute illness. Second, it tests whether a home exercise and thinking-skills program after discharge helps improve physical function, mental sharpness, and activity levels. About 200 older adults in Navarre will take part, with half getting the exercise program and half getting usual care.
