Summary
This study is testing whether patients who have had a minor stroke or TIA (transient ischemic attack) can be safely monitored and treated at home instead of staying in the hospital. Researchers want to see if this home-based program reduces the risk of another stroke, lowers hospital costs, and improves the patient's experience compared to standard hospital care. The program uses remote monitoring technology and requires patients to have a smartphone and family support.
