Summary
This study is testing whether adding a personalized, home-based exercise program to standard medication can help people with both chronic heart failure and kidney problems. Researchers will compare two groups of 30 patients each over six months: one group gets only standard drugs, while the other gets the same drugs plus a tailored exercise plan. The goal is to see if exercise safely improves heart and kidney function, fitness, and quality of life for these patients.
