Summary

This study is testing a new home care program called ADMIT that uses technology to coordinate health and social services for frail older adults. It aims to help them spend more days living safely at home instead of in hospitals or nursing facilities, while also improving their quality of life and reducing health crises. The program is being tested with 1,000 participants in Spain to see if it works better than the usual care they receive.
