Summary
This study is testing a new home care support program for people who have had surgery for colorectal cancer and now have a stoma. The goal is to see if better preparation and follow-up after leaving the hospital helps patients manage their care at home, feel more confident, and avoid unplanned trips back to the hospital. Researchers are comparing outcomes for 98 patients to understand what support works best.
