Summary
This study compares two approaches to help older adults with multiple chronic conditions manage high blood pressure. Researchers will test whether regular home blood pressure monitoring with standard clinic support works better or worse than the same monitoring with additional pharmacist support. The goal is to see which method more effectively lowers blood pressure and reduces the burden of treatment over one year.
