Summary
This study is testing a new health program designed to help older adults in Singapore stay independent and healthy as they age. It will screen 600 participants for early signs of physical and mental decline, then provide personalized support like doctor referrals and a health-tracking app. Researchers will compare an immediate support group to a delayed group to see if early help leads to better mobility and quality of life.
