Summary
This study surveys 5,000 people living with type 2 diabetes and multiple health conditions, along with their family members, to identify their health literacy strengths and needs. Participants will complete a one-time questionnaire about their ability to find, understand, and use health information. The goal is to improve support for patients and families managing complex health needs.
