Summary

This study tests a health chat program designed to help people with two or more heart and metabolic conditions (like high blood pressure, diabetes, or heart disease) improve their lifestyle and well-being. About 232 adults will use the program on WeChat to set goals, learn from videos, and talk with researchers. The goal is to see if the program is easy to use and can improve health behaviors, symptoms, and quality of life.
