Summary

This study tests a health chat program designed to help adults with two or more cardiometabolic diseases (like high blood pressure, diabetes, heart disease, or stroke) improve their lifestyle habits. About 240 participants will use a WeChat-based program for lifestyle planning and support, while a comparison group receives usual care. The goal is to see if the program is practical and can improve health behaviors, heart and metabolic measures, symptoms, quality of life, and physical function.
