Summary
This study tests a health chat program designed to help adults with two or more heart-related conditions (like high blood pressure, diabetes, or heart disease) make healthier lifestyle choices. Researchers will check if the program is easy to use and if it improves daily habits, health numbers, symptoms, and quality of life. The study involves 240 participants in Hong Kong who will either use the chat program or receive usual care.
