New care approach aims to cut kidney and heart risks in diabetes patients

Summary
This study tests whether a structured, goal-directed care plan can better manage kidney and heart health in people with diabetic kidney disease compared to usual care. About 100 adults with type 2 diabetes and kidney disease will be randomly assigned to either the new care approach or standard treatment. The goal is to see if this strategy helps more patients get the right medications and stick with them.
