Summary
This study tested a program called GET2HOME, which uses telehealth to help children with complex chronic conditions and their families after a hospital stay. The goal was to see if the program reduces urgent care visits, emergency room visits, or unplanned readmissions. 638 children and their families took part after being discharged from a children's hospital.
