First seen Jun 25, 2026 · Last updated Jun 26, 2026 · Updated 1 time
Summary
This study tests a geriatric telemanagement program for nursing home residents who have just been discharged from the hospital. A geriatrician leads a virtual team with the resident's doctor and nursing home staff to review diagnoses and treatments. The goal is to see if this approach can reduce hospital readmissions by at least 15% and improve care coordination.

If successful, this could provide a cost-effective model to reduce hospital readmissions and improve care for older adults transitioning from hospital to nursing homes.
