Can western patients skip surgery for early stomach cancer?
NCT ID NCT07781319
First seen Aug 24, 2026 · Last updated Aug 25, 2026 · Updated 1 time
Summary
This study looks at early gastric cancer, a disease where the tumor is confined to the stomach's inner layers. It asks whether the pre-surgery assessment (clinical staging) matches the actual findings after surgery (pathological staging) in a large group of Western patients. The research also checks if Asian guidelines for minimally invasive endoscopic removal are suitable for these patients, and tracks long-term outcomes like recurrence and survival. By analyzing data from 600 people in a Spanish registry, the goal is to improve treatment decisions and reduce unnecessary procedures.
What this could mean
Our plain-language read of the trial. This is informational only, not medical advice or a prediction.
- What this could lead to
- If successful, this could refine treatment decisions for early gastric cancer in Western populations, potentially reducing unnecessary surgeries or improving outcomes.
- What could go wrong
- This is a retrospective observational study, so it cannot prove cause and effect. Results may not apply to all patients or regions, and the analysis depends on the accuracy of existing medical records.

Who is studied
The study population consists of adult patients with a preoperative clinical diagnosis of early gastric cancer (cT1) who underwent curative-intent gastrectomy with lymph node dissection at participating centers of the Spanish EURECCA Registry between January 2014 and December 2022. The cohort includes patients with histologically confirmed gastric adenocarcinoma and complete clinicopathological and follow-up data available for analysis. This population was selected to evaluate the accuracy of preoperative clinical staging and to investigate the pathological characteristics and oncological outcomes of early gastric cancer in a Western multicenter cohort.

Inclusion Criteria: * Adults (≥18 years of age) * Clinical diagnosis of early gastric cancer (cT1) before surgery. * Curative-intent gastrectomy with lymph node dissection performed between * January 2014 and December 2022. * Histologically confirmed gastric adenocarcinoma. * Inclusion in the multicenter Spanish EURECCA Registry. * Availability of complete preoperative clinical staging, pathological findings, and follow-up data required for the study analyses. Exclusion Criteria: * Receipt of neoadjuvant therapy before surgery. * Incomplete pathological data precluding accurate tumor staging or lymph node assessment. * Non-adenocarcinoma gastric tumors.
