New VA program aims to help older veterans regain independence after hospital stays

Summary

This study is developing and testing a program called CTraC-FIT to help older veterans recover their physical function after a hospital stay. About 40 veterans aged 65 and older with some mobility problems will take part. The program adds functional assessments and goal-setting to an existing VA transitional care program. The goal is to see if the program is acceptable, feasible, and helps veterans regain independence.
