Summary
This study looks at whether a digital tool called Fragiclic helps doctors and nurses screen for frailty in people aged 65 and older during regular checkups. Frailty increases the risk of falls, hospital stays, and loss of independence. The goal is to see how often the tool is used and whether it reduces unplanned hospital visits or deaths. About 726 participants will take part, with no extra tasks beyond their usual care.
