Summary
This study tests a simple checklist to guide doctors, nurses, and care managers in helping people with advanced dementia and their families transition to hospice care. About 96 participants, including patients, their care partners, and healthcare workers, will try the new approach. The goal is to see if the checklist is practical and helpful, and whether it leads to smoother hospice enrollment and greater satisfaction for families.
