Street medicine team may keep homeless patients out of the ER

Summary

This study looks at whether giving homeless patients special follow-up care from a street medicine team can reduce repeat emergency room visits and hospital stays. About 390 homeless adults who are discharged from the ER will either get this enhanced support or the usual clinic follow-up. The goal is to improve health and lower the burden on emergency services.
