ER to street: can a simple referral keep homeless patients out of the hospital?

Summary
This study tests whether homeless adults who get a referral to a street medicine team after an ER visit have fewer repeat ER visits and hospital stays compared to those who get routine follow-up. About 390 unsheltered adults will take part. The goal is to improve access to primary care and reduce unnecessary hospital use.
