Summary
This study tested a new remote health system designed to help adults with multiple chronic diseases like heart failure, lung disease, and diabetes. The system used phone calls, virtual classes, and home monitoring to help patients manage their conditions. The main goal was to see if the system worked properly and was easy for patients and doctors to use, with the hope it could improve health and reduce hospital visits.
