Summary
This study is testing whether a smartphone app can help people better manage their heart health after having a stent placed. About 800 participants will either use the app for remote monitoring and guidance or receive their usual doctor's care. The main goal is to see if the app helps more people reach healthy targets for blood pressure and cholesterol over one year.
