Summary
This study is testing a new digital care program for people recovering from a heart attack. It trains pharmacists to provide remote support, helping patients manage medications, lifestyle changes, and vital signs from home. The goal is to improve recovery, prevent future heart problems, and reduce the need for hospital readmissions.
