Summary
This pilot study is testing a new digital health program designed to help adults manage multiple long-term conditions like diabetes, high blood pressure, and depression. The program includes a digital tool for doctors, a telemedicine system for specialist advice, and a mobile app for patients, all supported by community health workers. The main goal is to see if this combined approach is practical, easy to use, and acceptable to patients and healthcare providers in rural areas of India and Nepal before a larger trial.
