Summary
This pilot study will test a digital health tool designed to help adults aged 40 and older who have two or more long-term health conditions, like diabetes, high blood pressure, or depression. The tool includes an electronic system to guide doctors, telemedicine for specialist advice, and a mobile app for patients. The study will check if the tool is easy to use and works well in rural health centers in India and Nepal, involving about 180 patients and their healthcare providers.
