Summary
This study is testing a new digital support system to help people with type 2 diabetes. It combines a smartphone app for personalized lifestyle coaching with a computer tool that gives doctors treatment recommendations. The goal is to see if this integrated approach helps patients manage their diabetes better and lowers their risk of heart disease compared to standard care.
