Summary
This study aims to test a new community-to-clinic care strategy to help people with type 2 diabetes in Bangladesh better manage their condition. It will involve 5,000 adult residents in Sylhet city to see if combining community health workers with clinic services improves diabetes control compared to usual clinic care alone. The goal is to find a practical way to implement proven World Health Organization guidelines in a real-world urban setting.
