Summary
This study is testing a 4-week transitional care program designed to prevent burnout for caregivers and avoid crisis situations for people with disabilities. It will involve 100 participants, including both the person with a disability and their primary caregiver. The main goal is to see if the program successfully helps participants meet their personal care and support objectives, which could improve quality of life and reduce emergency care needs.
