Summary
This study is testing a new support checklist designed to help families and home healthcare workers guide loved ones with advanced dementia into hospice care. It aims to make this difficult transition smoother and more supportive for everyone involved. The research will check if the checklist is practical, helpful, and acceptable to use in real-world home care settings.
