New study aims to close the gap in cancer survivor Follow-Up care

Summary
This pilot study is testing whether extra support helps people who have had surgery for early-stage colorectal cancer get the recommended follow-up tests and scans. The support includes a patient navigator to help schedule appointments and reminders, an interactive website with education and checklists, or both. The goal is to see if these tools make it easier for survivors to complete the complex follow-up care needed to catch any cancer return early.
