Summary
This study will create a health management platform for people with early-stage Cardiovascular-Kidney-Metabolic (CKM) syndrome, a condition linking heart disease, kidney problems, and metabolic issues like diabetes. Researchers will enroll 150 adults aged 18-75 who can use a smartphone and wearable tracker. The platform will provide personalized diet and exercise plans, monitor health data, and offer follow-up care over two years to see if it improves health and quality of life.
