Summary
This study compares standard postpartum care to a program called CAREPATH, which includes three telehealth visits with a community health worker. Researchers want to see if this approach helps more new mothers complete their postpartum checkup, catch complications early, and avoid emergency room visits or hospital readmissions. Up to 500 pregnant individuals aged 16-44 who speak English, Spanish, or Marshallese can join.
