Summary
This study will test whether a comprehensive care coordination program helps high-risk Medi-Cal members in California avoid unnecessary hospital stays and ER visits. About 1,200 adults who recently left a hospital or other care facility will receive extra support like care management, housing help, and transition services. The goal is to see if this approach improves health outcomes and reduces costs.
