Summary
This study looks at whether closer collaboration between hospital and community pharmacists can reduce drug-related problems when patients leave the hospital. It involves 150 adults who take seven or more medications and are being discharged home. A hospital pharmacist shares medical info with the patient's usual pharmacy to help them handle complex prescriptions more safely.
