Summary

This study looks at whether having a hospital pharmacist share medical details with a patient's regular community pharmacist can reduce mistakes when the patient leaves the hospital. It involves 150 adults who take 7 or more medicines and are going home from an internal medicine ward. The hospital pharmacist does not work directly with the patient but only with the community pharmacist to make discharge prescriptions safer.
