Summary
This study tested whether a simple, one-page discharge summary could improve the transition from hospital to home for older patients. 111 older adults with multiple health conditions received either this new tool or usual care. The tool summarized key info like medications and warning signs, and staff used a special teaching method to help patients and caregivers understand and remember it.
