This study looks at whether community health workers can deliver a combined program to help people with both common mental health problems (like anxiety and depression) and chronic diseases (like high blood pressure or diabetes). The program includes stress reduction, behavioral activation for depression, and motivational interviewing to encourage healthy habits.
