Summary
This study tests a new way to care for people after a heart attack. It uses specially trained pharmacists and digital tools like mobile phones to help manage health after leaving the hospital. The goal is to reduce the chance of another heart attack or stroke and improve overall well-being. About 1,600 adults who had a heart attack are taking part.
