This study tests a program called AHEAD that helps young people aged 16-19 with type 1 diabetes improve their blood sugar control as they move from pediatric to adult diabetes care. About 306 participants will either get usual care or the AHEAD program, which offers tailored support to build independence. Researchers will compare changes in blood sugar levels and how participants feel about their diabetes.
