Summary
This study is testing whether a special support program helps young adults with type 1 diabetes manage their blood sugar better during the challenging transition from pediatric to adult care. Researchers will compare the new program, which provides tailored coaching to build independence, against standard care. They will measure changes in blood sugar control and how participants feel about managing their diabetes.
