Summary

This study tests a new home care program for frail older adults (70+) who have complex health and social needs. The program uses technology and teamwork between health and social services to help people stay at home longer and avoid emergencies. About 1,000 participants will be followed to see if the program improves days at home, quality of life, and reduces hospital visits.
