New study tests Tech-Powered home care to keep seniors out of hospitals

Summary
This study is testing a new home care program that uses technology and teamwork between health and social services to help frail older adults stay at home longer. About 1,000 people aged 70 and older who live at home and have complex health or social needs will take part. The goal is to see if the program increases the number of days they spend at home and improves their quality of life compared to usual care.
